Prepare the Personal Statement with Important Information
There is a statement form you need to fill based on the above regulation, yet it is available in Indonesian language. Not all foreigners understand it. To help you with, here’s some tips of information you need to include (prepare it and print it out for your personal information, it will benefit you).

Address : 1. (address in your country) 2.(address in Indonesia. Put hotel address if you stay in hotel)
ID Number: (your passport number)
Name of Vessel or Vessel Number: (your flight name)
Date of arrival: (it’s obvious right?)
Name and User’s address: (put the name if the medicine is not used by you, and the user address in Indonesia if the user is not you). (if you are the user, then write the same name and Indonesian address you are staying)

Custom Declaration Form
The personal statement above is not mandatory yet, it is beneficial if you prepare it. Custom declaration most likely has to be filled. The custom declaration form will be given approximately an hour or two prior to landing. You have to declare the medicine you bring through the form. Particularly for medicine, you have to show a prescription or doctor’s recommendation.
